Access constraints continue to limit independent monitoring
Where monitors cannot travel, violations go unrecorded — and the absence of a record is later read as the absence of an event.
Independent monitoring depends on being able to reach people. Where that is not possible, documentation relies on remote methods that take longer to verify and carry more risk for the person supplying the information.
We publish what we can substantiate and are explicit about what we cannot reach. A gap in our record is a gap in access, not a finding that nothing occurred.
